With the passage of the Paperwork Burden Reduction Act (H.R. 3797), employers are no longer required to provide hard copies of form 1095-C without request by the employee. If you are/were a full-time employee of Seasons or Crown Hospice and would like a copy of this form, you may make a request from Human Resources:
